Lifestyle Empowerment Approach for Diabetes Remission (LEADR) is a lifestyle medicine program delivered through a shared medical appointment (SMA) model that helps clinicians guide patients toward type 2 diabetes remission. LEADR is a transformative leadership development program designed to cultivate purpose-driven leaders who align personal well-being with professional impact. Rooted in the principles of lifestyle medicine, the LEADR curriculum integrates evidence-based health behaviors with essential leadership skills to support sustainable performance, resilience, and meaningful change. Through experiential learning, reflection, and practical application, participants are equipped to lead with clarity, authenticity, and influence—both within their organizations and in service of the communities they impact.

The adaptable curriculum supports in-person, virtual, or hybrid delivery. Reimbursement-focused pilot testing is underway.

How To Use This Program

The LEADR Program is designed for flexible delivery over 12 weeks—in person, online, or in a hybrid format—the framework includes a comprehensive Facilitator Script and Participant Guides that help ensure consistent implementation and strong patient outcomes.

The LEADR Program is for: 

  • Health systems and organizations participating in approved pilots 
  • Interdisciplinary care teams and lifestyle medicine–trained facilitators
  • Program participants enrolled through an approved site

LEADR is not a self-directed, consumer-based program; it is delivered in structured, facilitated environments.
