Purpose of LEADR

Lifestyle Empowerment Approach for Diabetes Remission (LEADR) is a structured, group-based program that supports adults with type 2 diabetes through evidence-based lifestyle medicine interventions.

LEADR emphasizes nutrition-centered behavior change, participant empowerment, and clinician-facilitated shared medical appointments. The program is scalable, flexible, and designed to foster a supportive community of participants.

Facilitator Role & Responsibilities

LEADR facilitators serve in a dual role as both program facilitator and clinical guide.

  • Guide participants through each session using the facilitator script or your own words
  • Create a nonjudgmental, supportive, and engaging environment
  • Encourage participant sharing while maintaining appropriate boundaries
  • Monitor time and keep sessions on track
  • Redirect discussion when conversation moves off-topic
Facilitators are encouraged to adapt delivery style while maintaining fidelity to LEADR’s core principles and learning objectives.
Clinical Oversight & Safety Considerations

LEADR is intended to be taught or overseen by a physician or advanced practice provider (APP) with prescribing authority.

  • Participants may experience rapid improvements in blood glucose levels
  • Medication adjustments may be required
  • Frequent or continuous glucose monitoring is recommended, especially during the first four weeks
Participant safety should remain the primary consideration throughout the program.
Billing & Documentation Notes

Many organizations deliver LEADR as a shared medical appointment (SMA).

  • Clinicians must have one-on-one clinical interactions with each participant
  • These interactions are ideally incorporated during the “Set Your SMART Goals” segments
  • SMART goal segments begin in Session 2 (Session 1 does not include individual goal setting)
Facilitators should coordinate with their organization’s billing and compliance teams to ensure documentation requirements are met.
Program Delivery Formats

LEADR sessions may be delivered:

  • In person
  • Virtually
  • Using a hybrid format

When script language differs by format, sections are clearly labeled in the facilitator materials.

Family members are welcome to participate when appropriate and feasible.
Teaching Kitchen Options

LEADR includes 12 Teaching Kitchen segments that can be delivered flexibly based on time, space, and available resources.

  • Live cooking demonstrations during sessions
  • Food prepared in advance with tasting and discussion
  • Education-focused walk-throughs without food preparation

Recipes and shopping lists are included in participant materials and may be used as handouts, homework assignments, or optional challenges.

  • Assign select content between sessions if time is limited
Prioritize participant engagement, understanding, and sustainability over rigid completion of all content during live sessions.
