What Is LEADR?

LEADR (Lifestyle Empowerment Approach for Diabetes Remission) is a supportive, group-based program designed to help adults with type 2 diabetes improve their health through evidence-based lifestyle medicine.

This program focuses on nourishment, education, community, and practical behavior change strategies that support long-term health.

What to Expect During the Program

  • Evidence-based nutrition education
  • Group discussion and peer support
  • Practical tools you can use in everyday life
  • Time to reflect, set goals, and track progress
LEADR is not about perfection — it’s about progress, support, and sustainability.

You are an active partner in your health journey throughout LEADR.

  • Attend sessions and participate to the best of your ability
  • Engage in discussions and activities
  • Practice skills and strategies between sessions
  • Communicate with your facilitator if challenges arise

Your Health Vision & Goals

A core part of LEADR is clarifying what better health means to you. You’ll have opportunities to reflect on your health vision and set realistic, meaningful goals.

Beginning in later sessions, you’ll work with your facilitator to set SMART goals — goals that are Specific, Measurable, Attainable, Realistic, and Time-based.

Teaching Kitchen & Food Education

LEADR includes Teaching Kitchen experiences that help you explore nourishing food choices in a practical, approachable way.

  • Cooking demonstrations or guided food education
  • Recipes and meal ideas
  • Tips for eating well on a budget

Participation may look different depending on your session format, but all activities are designed to be inclusive and achievable.

Medical Care & Safety

LEADR supports — but does not replace — your medical care.

As lifestyle changes occur, your healthcare provider may need to review medications or monitor health markers.

Always follow the guidance of your healthcare provider and communicate any concerns during the program.
